Student Travel Sheet

Please use the following form to make a request for either domestic or international travel for exchange students participating in Student Ambassador Exchange Programs. All travel must be approved by SAE Headquarters prior to booking. If students plan to travel internationally, they MUST mail the DS-2019 by CERTIFIED MAIL to SAE Headquarters for approval to travel internationally. Students participating on certain visas may not be able to travel internationally while participating in the program. The address to send DS-2019 for travel is:

Student Travel Sheet

Students and families should use this form to seek approval for international visitors coming to the United States to visit a student participating in SAE's exchange programs. Visitors must secure SAE Headquarters approval prior to visiting a student in the program. Please write down the student visitor’s information and proposed itinerary then have all necessary parties sign at the end of the document.
  • If the student is traveling alone, there must be an adult present at the destination location responsible for the student. Please list the name, phone number, and email for the responsible adult.
